Reference Interval:
|
Measure trough level after at least 5 days administration and aim for >=1 mg/L for treatment and >=0.7 mg/L for prophylaxis. Consider ongoing therapeutic drug monitoring in the event of clinical failure, toxicity, change in dose or formulation, new interacting medications, or conditions leading to poor absorption. For clinical advice, discussion with an Infectious Disease Physician, Clinical Microbiologist or Clinical Pharmacist is encouraged.
