Samples to be collected at PathWest RPH or Fremantle Hospital only.
Samples are collected at PathWest Goderich Street (Mon-Thurs 0730-1700, Fri 0730-1200) and PathWest Fremantle Hospital (Mon-Thurs 0730-1400).
Ensure CTTWA Request for the Preparation & Supply of Autologous Serum Eyedrops (FORM94) is signed by Requesting Doctor within the last 12 months.
Ensure CTTWA Consent for Collection of Blood & Preparation of Autologous Serum Eyedrops (FORM67) is signed by Patient and Requesting Doctor within the last 12 months.
Enclose pharmacy paperwork with samples (if present). |

Samples arriving at CSRA-RPH after CTTWA operating hours Monday - Thursday are to be stored overnight in CSRA at ambient temperature.
Do not centrifuge or aliquot.
Notify CTTWA as soon as possible. |
