Clinical Use:
|
Consider TDM in all patients receiving linezolid and if >14 days therapy is planned (weekly if stable) given significant haematological adverse effects. Strongly recommended in patients with impaired renal function or undergoing dialysis and for elderly patients.
Measure trough concentrations 3 days after commencement of therapy. |
