Requests are valid for up to 6 uses within a 6 month period from the initial request date. INR tests are an exception and can be collected as often as needed within this 6 month period. Patients must meet the specific Rule 3 tests/therapy/condition requirements to qualify. Any non-qualifiable tests requested will only be resulted once. |
