Tissue Transglutaminase Antibody


General
  Clinical Use: IgA anti-transglutaminase antibodies (IgA-TTG) are directed against the major coeliac disease autoantigen and is the first line test for coeliac disease.
In people on a gluten-containing diet, the presence of IgA anti-TTG antibodies is highly sensitive and specific for coeliac disease (>95%).
IgA-TTG antibody level can assist in monitoring response to gluten-free diet in patients with coeliac disease.
Note: Total serum IgA is performed and result reported together with IgA-TTG. This is because i) patients with coeliac disease have a higher incidence of IgA deficiency and ii) sensitivity of IgA-TTG testing is reduced in patients with low serum IgA levels. Patients with total serum IgA levels < 0.2g/L will be tested for IgG antibodies to deamidated gliadin.
Due to the superior positive predictive value of IgA-tissue transglutaminase and IgG-deamidated gliadin antibodies for coeliac assessment, PathWest does not offer and will not refer testing for endomysial antibodies.
