PML-RARA Quantitative Real Time PCR


General
  Clinical Use: These real time quantitative PCR assays for each of the t(15;17) breakpoints (BCR1, 2, 3) allow the exact measurement of a patients' level of disease by calculating the number of copies of t(15;17) (ie PML-RARA fusion gene transcripts). Transcript levels less than 10 are considered below the detection limit of the assay and are called negative but are reported as <10 to indicate that some amplification occurred. Transcript levels that do not amplify at all are denoted a value of zero. A control gene (ABL) must amplify a predetermined number of copies (depending on the patients level of disease) to qualify that the sample is of sufficient quality and quantity to validate the result. A recollect is recommended if the control gene fails and a "no result" issued. Patient % ratio results are most informative when considered serially, observing the change in level of disease over time. Note that up to a 2-fold change in consecutive results is considered normal variation of the assay, a 2-10-fold increase suggestive of impending relapse (retest to confirm and monitor patient more closely) and a >10-fold increase defines molecular relapse.

Results are reported as a normalised ratio of the number of copies of PML-RARA transcripts divided by the number copies of the control gene (ABL) detected in a defined volume of cDNA.  Results are expressed as a percentage ratio.
