BCRABL Kinase Domain Mutation Studies


General
  Clinical Use: Specimens are referred to SA Pathology for testing.
This test detects mutations of the ABL kinase domain that may have occurred in response to therapy with tyrosine kinase inhibitors (TKI) conferring resistance located at amino acids 240-490. Note, that ABL kinase domain mutations are not the only mechanism of TKI resistance. Criteria for testing: Primary resistance - failure to achieve major cytogenetic response (MCR) or BCRABL% <10% by 6 months of imatinib. Secondary resistance - loss of partial or complete haematological response, loss of MCR, clinical progression to accelerated phase or blast crisis or significant increase in BCRABL% (>2-fold increase). The lab may request that small increases in disease level are confirmed in a second sample. Discussion with the clinician must also occur to ensure no compliance issues or dose changes.
